The Qualified Entity Program


Columbia Data Analytics is certified as a Qualified Entity (QE) and conducts public reporting in accordance with QE Program requirements and CDA’s approved reporting framework. We have combined CMS Medicare Fee-for-Service enrollment and claims data (Parts A, B, and D) with linked, de-identified Kythera Labs dataset to assess care delivery and outcomes for individuals diagnosed with heart failure (HF) and acute myocardial infarction (AMI).


Measures related to hospitalization, medication use after discharge, and subsequent utilization were measured. In this report, CDA focuses on beneficiaries with heart failure or acute myocardial infarction and assesses treatment initiation, adherence, persistence, and readmission outcomes. The purpose is to present clear, understandable regional performance information for consumers, providers, health plans, researchers and other stakeholders.

Data Sources

The analysis uses linked Medicare Part A, Part B, and Part D claims together with Medicare enrollment files and the Kythera Labs population, including inpatient, outpatient, professional, pharmacy, and enrollment information.
All data used to create the Columbia Data Analytics QE public report are de-identified in compliance with the Health Insurance Portability and Accountability Act (HIPAA).
